Teen Activity Card Application Form [PDF]


This year, there are three TEEN ACTIVITY CARDS OPTIONS available:


Teen Standard Activity Card Membership: $25
  • participate in any of the activities listed in the Drop-in Activity Schedule [PDF] for no additional cost, plus other upcoming drop-in activities at the new Eyer Wideman House;

  • pay a reduced fee for most youth special events;

  • meet new friends with common interests;

  • receive a 5 per cent discount on the following registered Youth Programs: Kickboxing, all Art Programs, all Dance Programs and Table Tennis Programs; and

  • be entered into sessional draws for prizes, such as mall gift certificates and much more!


    Teen Standard Plus Dropin Sports Membership: $5
  • participate in any of the activities listed in the Drop-in Activity Schedule [PDF] and pay only $2 per drop-in acitivity, plus other upcoming drop-in activities at the new Eyer Wideman House;

  • pay a reduced fee for most youth special events;

  • meet new friends with common interests;

  • receive a 5 per cent discount on the following registered Youth Programs: Kickboxing, all Art Programs, all Dance Programs and Table Tennis Programs; and 

  • be entered into sessional draws for prizes, such as mall gift certificates and much more!


    Pay As You Go Option
  • If you do not wish to purchase an Activity Card, you can participate in any of the drop-in activities listed in the Drop-in Activity Schedule [PDF] for a cost of only $4 per activity.



    HOW TO GET A TEEN ACTIVITY CARD
    Teen Activity Cards can be obtained at the main Reception area of your local community centre. But first, complete the following steps: 

    1) Download and fill out the Application Form (see above). Application forms can also be picked up at any Richmond Hill community centre.

    2) Take your completed form to the main Reception area of your local community centre.

    3) Hand in your completed form and provide photo I.D. and payment to the receptionist.

    4) Get your membership picture taken and receive your temporary card.

    5) You will receive a phone call when your permanent card is ready. When you come in to pick up your card, return your temporary card and your permanent card will be issued.

    Please note: Youth must be in grades 6 to 12 to attend Drop-In Programs. 



    FREQUENTLY ASKED QUESTIONS
  • Does the Teen Activity Card expire?
    Yes, the card will automatically expire at the end of April. However, hold on to your card, as you can still use it next year when you renew your card.

  • Do I have to bring my Teen Activity Card every time I attend a Drop-In Program?
    Yes, it is important that you bring your card every time. Otherwise you will not be granted admission in to the program.

  • When will I get my Teen Activity Card?
    Once the form has been handed in with payment, you will receive a temporary card. Your permanent card will be mailed to you.

  • Do I have to wait for my Teen Activity Card before I can participate in a Drop-In program?
    No, you will receive a temporary card when you drop-off your application with payment. This will allow you to attend programs immediately.

  • Can I use my Teen Activity Card at any Richmond Hill Community Centre?
    Yes, your card can be used at any of the community centres for Youth Drop-In Programs.

  • Can I use my card to admit my friends?
    No, each individual must have their own Teen Activity Card, including family members.

  • What happens if I lose my Teen Activity Card?
    If your card is lost or stolen the purchaser is responsible for replacement at full cost.
