The 50th Annual Volunteer Achievement Awards Nomination Form

Criteria for submitting this form:
If you would like to be nominated, please have a nominator complete and submit a Nomination Form for you. As a nominee, you are not eligible to nominate yourself.
Please include two Letters of Reference for each Nomination and a brief Biography (details noted below).
Please be sure to complete your Volunteer Achievement Award Nomination Form in full; incomplete forms will not be accepted.
The deadline for 50th Annual Volunteer Achievement Awards nominations is Friday, February 24, 2012.
Eligibility Requirements:
Nominee must be 18 or older.
A minimum of five consecutive years of volunteer service is required (35 hours per year).
The nominee must be a Richmond Hill resident.
Nominees must not have received payment for their volunteer work or performed the services as part of their regular business or professional duties.
Nominees must be active beyond simple membership in the organization.
