Certificate of Insurance Forms
Individuals and organizations who supply goods and services to the City or who use City facilities may be required to provide evidence of insurance as part of the terms and conditions of the contract, purchase order, permit, license or other forms of agreement.
Evidence of insurance will ONLY be accepted on a City of Richmond Hill Certificate of Insurance Form which must be completed by a certified insurance broker or authorized representative of the insuring company. For assistance completing the City Forms, please refer to number 1 and 2 below - “Instructions for Insurance Brokers”.

This form is used to provide evidence of liability insurance by Special Events Organizers and Participants taking place within the City of Richmond Hill and for City facility rentals.
